Forms and Waivers

To ensure a clear and communicative experience, one parent/guardian per student is responsible for understanding the policies and procedures at Reno Ripples. The parent/guardian must also agree to and sign a waiver and release form before lessons can begin. In an effort to reduce our carbon footprint, all forms are electronic.

Reno Ripples Swim Liability Waiver

PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS. I, (insert full name below)

the enrolled participant and/or the parent/guardian of the participant agree and understand that swimming is a hazardous activity. I recognize that the inherent risk of the sport may include but is not limited to physical or emotional injury, paralysis, death, or other damage to self.
